Membership Application and Renewal
Membership year runs annually from July 1st to June 30th.
- INDIVIDUAL MEMBERSHIP = $ 15.00
- JOINT MEMBERSHIP = $ 25.00
Two members living at the same address will receive only one mailing.
- STUDENT MEMBERSHIP = $ 10.00
Application/renewal form must be accompanied by photocopy of a valid Student I.D.
Please return the form with payment, checks should be made payable to “Philadelphia Chapter SAH”, to:

Dues cover operating expenses including mailings. Some programs may require additional fees. You may join at any time but the membership will only be valid through the following June 30th, at which time you will have to renew. Please look at the date on the top of the mailing label on the envelope your renewal form was mailed in. If it shows 2018 you have already renewed for 2017-2018, if not please submit this form with your dues.
