Membership Application and Renewal
Membership year runs annually from July 1st to June 30th.
- INDIVIDUAL MEMBERSHIP = $ 15.00
- JOINT MEMBERSHIP = $ 25.00 (two members at the same address)
- STUDENT MEMBERSHIP = $ 10.00
Application/renewal form must be accompanied by photocopy of a valid student I.D.
Please return the form with payment, checks should be made payable to “Philadelphia Chapter SAH”, to:

Dues cover Chapter operating expenses. Some programs may require additional fees. You may join at any time but the membership will only be valid through the following June 30th, at which time you will have to renew.
MEMBERSHIP RENEWALS MUST BE RECEIVED BY OCTOBER 1 TO REMAIN ON OUR MAILING LIST
